Thank you for your interest in our low cost spay and neuter program for pet cats.
At this time, vouchers are in very high demand. Completing this form does not guarantee a voucher, but it puts you in line. We will contact you on a first-come first-served basis when vouchers become available.

Important information - please read before submitting:
• Co-pay when collecting voucher: $65 per male cat and $90 per female cat.
• The voucher covers spay or neuter, microchip, and Rabies + FVRCP vaccines at River Oak Veterinary Hospital in Riverbank, CA.

• You will pick up your voucher in person at the Cat Network of Stanislaus office in central Modesto. When your turn comes up, we will tell you where and when to come in to pay for and collect your voucher.

Using the voucher

• You will need to call River Oak Veterinary Hospital to book your appointment only after you have your voucher in hand. You must use the voucher within one month of purchase.
• You must present the voucher on arrival at your appointment to receive services.
• Any additional fees charged by the veterinarian (such as pregnancy, heat, cryptorchid, flea treatment, or any medical concerns) are your responsibility. Tell the vet in advance if you suspect your cat is pregnant, in heat, or a cryptorchid male so you can confirm expected charges.
